Spanning the intercostal spaces of the male thorax, the internal intercostal muscles appear as thin, segmented slips running between adjacent costae from the costochondral region toward the posterior rib angles. In an anterior view of the skeleton, the ribs and sternum form the superficial framework, while the internal intercostals occupy the deep plane to the external intercostals and align with the internal surface of the thoracic cage. Their fibers course obliquely, generally inferoposterior, creating a consistent pattern across multiple rib spaces. Bony landmarks remain clear, including the manubrium and sternal body, costal cartilages, and thoracic vertebrae with the posterior rib articulations. Clinically, this layer matters because it defines the mechanics of forced expiration and stabilizes the intercostal spaces during coughing and exertion. Fiber direction and depth are also relevant for chest wall procedures: intercostal neurovascular structures run in the costal groove on the inferior border of each rib, between the internal intercostal and innermost intercostal layers, so an appreciation of where the internal intercostals sit helps explain why thoracostomy and needle decompression target the superior margin of a rib. Pain from intercostal muscle strain can mimic pleuritic or cardiac symptoms. Localization is everything.
