Description
Arising from the venous plexus along the medial border of the foot, the medial marginal vein courses anterior to the medial malleolus and continues proximally as the great saphenous vein. Superficial to the tarsal and metatarsal bones, it tracks along the medial aspect of the first metatarsal and hallux phalanges, while deeper plantar and dorsal structures remain implied by their skeletal landmarks. Nearby cutaneous nerves, typically the saphenous nerve on the medial ankle and the medial dorsal cutaneous branches over the dorsum, are shown in relation to the superficial venous channel. Orientation is clear. Medial stays medial. Understanding this segment matters because it anchors surface anatomy for venous access and for mapping superficial venous disease, from varicosities of the great saphenous system to reflux patterns traced back to the saphenofemoral junction. At the ankle, the vein’s close relationship to the medial malleolus and saphenous nerve explains why saphenous vein cutdown, cannulation, or harvest can be complicated by nerve irritation, hematoma, or difficult hemostasis in the thin subcutaneous tissue.
