Description
Along the lateral border of the male foot, the lateral marginal vein courses superficially from the region of the fifth toe toward the base of the fifth metatarsal, then continues proximally to feed the small saphenous vein posterior to the lateral malleolus. Deep to it lie the fifth metatarsal and cuboid, with the calcaneus forming the posterior bony contour and the distal tibia and fibula framing the ankle mortise superiorly. Red arterial channels track through the dorsum and plantar aspect in parallel, while light blue cutaneous nerves accompany the superficial venous pathway near the lateral margin. Spatial relationships are clear: vein superficial, metatarsals deep, and the lateral malleolus and peroneal tendon corridor just proximal and posterior. Clinically, this lateral marginal venous route is a common landmark when teaching superficial venous drainage of the foot and its transition into the small saphenous system. It also intersects a high yield pain generator: the base of the fifth metatarsal, where avulsion (zone 1) fractures and Jones fractures (zone 2) occur, and where swelling can obscure superficial veins and nearby cutaneous nerves on exam. Surface anatomy matters here. The illustration helps correlate palpation points, edema patterns, and the superficial venous network that may become prominent with venous insufficiency or after ankle and hindfoot immobilization.
