Description
Deep to the reflected trapezius and splenius capitis, the medial portion of semispinalis capitis is exposed along the posterior cervical midline, running superiorly to the occiput near the inferior nuchal line. Its fibers ascend from the cervicothoracic region (classically C7 to T6 transverse processes) and converge as a thick column that sits medial to the more obliquely oriented splenius group and posterior to the cervical multifidus and semispinalis cervicis. The nuchal ligament and spinous processes form the medial backdrop, while the muscle’s superior expansion approaches the suboccipital region beneath the occipital scalp. Layer relationships are clear. This medial semispinalis capitis view matters when you need to explain extension and contralateral rotation of the head without confusing it with the superficial trapezius. Pain referral from semispinalis capitis trigger points is a common contributor to cervicogenic headache patterns, and the muscle is frequently encountered during posterior cervical approaches when splitting the midline raphe and elevating paraspinal muscles off the spinous processes.
