Description
Seen from a posterior perspective through a semi-transparent lower back, the pancreas lies retroperitoneally across the upper abdomen, with its head seated in the C-loop of the duodenum on the patient’s right and the tail extending leftward toward the splenic hilum. Superior to the gland, the splenic artery typically tracks along the superior border toward the tail, while the splenic vein runs posterior to the body and joins the superior mesenteric vein to form the portal vein near the neck of the pancreas. Anterior relationships are implied by depth, with the stomach and lesser sac interposed, while posteriorly the gland abuts major vessels and the left kidney region. Orientation is clear. Because the view is posterior, it emphasizes the pancreas as a retroperitoneal organ and clarifies why posterior vascular relationships dominate both imaging interpretation and operative risk. This is the anatomy behind post-pancreatitis splenic vein thrombosis with left-sided portal hypertension and gastric varices, and it also frames the common site of pancreatic ductal adenocarcinoma in the head where a mass can compress the distal common bile duct and produce painless obstructive jaundice. For surgery, the posterior relations help explain why injury to the splenic vessels and portal confluence is a constant concern during distal pancreatectomy and pancreaticoduodenectomy.
