Cervical pars interarticularis (articular pillar) occupies the narrow osseous bridge between the superior and inferior articular facets of a typical cervical vertebra. From an oblique posterolateral perspective, the superior articular facet sits superoposterior to the inferior facet, with the pars forming the constricted segment between them and transitioning laterally into the lateral mass while remaining posterior to the vertebral body and uncinate processes. The adjacent lamina lies medial and posterior, and the pedicle courses anterolaterally toward the vertebral body, framing the intervertebral foramen just anterior to the articular pillar. Fine cortical margins emphasize how limited the bony cross-sectional area can be at this junction. Small, but clinically unforgiving. Fractures through the pars interarticularis define several high-yield injury patterns in the cervical spine, most famously the C2 pars defects of traumatic spondylolisthesis (Hangman fracture), where disruption between the facets destabilizes the posterior elements while threatening the spinal canal and vertebral artery corridor. For surgeons placing lateral mass screws, the articular pillar is also the working bone stock, and appreciating the pars-facet relationship helps explain why screw trajectory is planned relative to the superior articular surface and why facet violation can occur. The fixed viewpoint clarifies the step-off between the facets and the pars, a relationship that can be hard to internalize from axial CT slices alone. Use this illustration in gross anatomy and spine modules when teaching the cervical vertebrae, facet joint orientation, and the concept of the articular pillar, or in orthopaedic and neurosurgical publications discussing Hangman fracture classification, lateral mass fixation, and posterior element injury mechanisms.
