Description
Posterior lower leg anatomy is presented with the gastrocnemius forming the superficial calf contour while the soleus lies deep to it, broad and flat against the posterior tibia and fibula. Proximally, the medial and lateral heads of gastrocnemius arise superior to the soleus near the femoral condyles, then descend to join the calcaneal (Achilles) tendon. Distally, the common tendon narrows as it approaches the posterior calcaneus, with the ankle and subtalar region partially included for orientation. Bone landmarks, including the tibial shaft and fibular shaft, frame the muscular compartments laterally and medially. For teaching function, this relationship matters because gastrocnemius crosses both the knee and ankle, while soleus acts only across the talocrural joint, so plantarflexion strength and endurance change with knee position. That distinction underpins the Silfverskiöld test when assessing equinus contracture and helps separate a true Achilles tendon contracture from isolated gastrocnemius tightness. Strain patterns follow the same anatomy: gastrocnemius injuries cluster near the musculotendinous junction during explosive knee extension, whereas soleus strains and deep posterior compartment pain often present with less obvious surface deformity.
