Description
Running as a narrow vertical column along the posterior neck, the spinalis cervicis sits medial to the longissimus cervicis and immediately adjacent to the cervical spinous processes. Superiorly its fibers approach the axis (C2) and adjacent upper cervical vertebrae, while inferiorly they blend toward the upper thoracic region near the spinous processes and the cervical thoracic junction. A lateral view of the skull and upper torso also brings the occipital region, mandible, cervical vertebral bodies with intervertebral discs, and the first ribs and clavicle into orientation, with the scapula partially visible posteriorly. Deep and close to bone. Because spinalis cervicis is the most medial component of the erector spinae in the neck, this perspective helps clarify how extension and ipsilateral stabilization are generated without confusing it with the more superficial trapezius or splenius capitis. That distinction matters in posterior cervical approaches, where midline dissection tracks along the nuchal ligament and spinous processes and the surgeon works through the paraspinal muscle envelope to reach the laminae, facets, or interlaminar spaces. Strain and denervation patterns after whiplash, chronic postural loading, or multilevel cervical fusion can preferentially affect these deep extensors, contributing to loss of cervical lordosis and persistent axial neck pain.
