Description
Running longitudinally along the posterolateral cervical and upper thoracic spine, the iliocostalis cervicis is placed lateral to the thoracic spinous processes and medial to the ribs’ angles, forming the lateral column of the erector spinae group. Its slips arise from the angles of the upper ribs (classically ribs 3 to 6) and ascend superiorly to the transverse processes of the mid cervical vertebrae (C4 to C6), deep to the superficial thoracolumbar fascia and adjacent intrinsic back muscle layers. Lateral to the vertebral column, the levator scapulae tracks from the superior angle and medial border of the scapula up to the transverse processes of C1 to C4, bridging the shoulder girdle to the neck. Bone landmarks anchor the composition. Posterior cervical muscle anatomy gets clinically relevant fast. Iliocostalis cervicis is a frequent contributor to extension and ipsilateral lateral flexion, and its overuse patterns can present as cervicothoracic junction pain that worsens with prolonged head-forward posture or repetitive overhead work, often blending with myofascial trigger points in the broader erector spinae mass. Levator scapulae tightness drives scapular elevation and downward rotation, a common finding in neck pain with shoulder girdle dysfunction, and it is a targeted structure in manual therapy, dry needling, and botulinum toxin planning for certain cervical dystonia patterns. Palpation and needle trajectories depend on knowing what lies deep and medial.
