Description
Running along the posterolateral neck, the iliocostalis cervicis appears as narrow, segmented fascicles forming the lateral column of the erector spinae, positioned lateral to the cervical spinous processes and medial to the posterior axillary fold. Superiorly, its slips reach toward the transverse processes of the mid cervical vertebrae (classically C4 to C6), while inferiorly it blends into rib attachments in the upper thorax, near the angles of ribs 3 to 6. Deep bony landmarks include the occipital bone, the seven cervical vertebrae, the upper ribs, and both scapulae, which frame the muscle bed on either side of the vertebral column. Levator scapulae is shown nearby, coursing from upper cervical transverse processes down to the superomedial scapular border. Clinically, this region matters because the iliocostalis cervicis and adjacent erector spinae segments are common generators of posterior neck pain and cervicogenic headache patterns after overload or whiplash, and they are frequent targets for myofascial trigger point examination. Palpation and dry needling often reference its lateral position relative to the cervical laminae and facet joints, where dorsal rami innervation and segmental tenderness can help differentiate muscular pain from radicular symptoms.
