Description
Running bilaterally along the posterior cervical and upper thoracic spine, the iliocostalis cervicis appears as the lateral cervical component of the erector spinae, positioned lateral to the spinous processes and medial to the posterior axillary line. Superiorly, its slips converge toward the transverse processes of C4 to C6, while inferior fibers arise from the angles of the upper ribs (classically ribs 3 to 6) and blend with adjacent erector spinae fascia. Medial to it lies the longissimus cervicis and, deeper still, semispinalis cervicis approaching the midline; superficial layers such as trapezius and splenius capitis may be reflected or rendered in neutral tone to keep the segmented iliocostalis pattern readable. Scapular outlines sit laterally, framing the upper thoracic region. Posterior labeling of iliocostalis cervicis matters because it is often confused with splenius muscles in surface anatomy and in teaching atlases, yet its fiber direction and attachments explain its actions and pain patterns. It extends and ipsilaterally laterally flexes the cervical spine, and overuse in sustained desk posture or guarded breathing can produce focal myofascial tenderness near the upper rib angles. In posterior cervical approaches and facet-based procedures, knowing the relative positions of the erector spinae columns helps anticipate muscular planes encountered during midline versus paramedian dissection and reduces inadvertent denervation or unnecessary stripping from the transverse processes.
