Description
Running along the apices of the lumbar spinous processes, the supraspinous ligament (ligamentum supraspinale) forms a dense fibrous band that links adjacent vertebrae from L1 through L5 and continues inferiorly toward the sacral spinous region. In a lateral lumbar spine perspective, its fibers sit posterior to the vertebral arches and spinal canal, superficial to the interspinous ligament and closely applied to the tips of the spinous processes. Anterior to it, the intervertebral discs occupy the space between vertebral bodies, while posterior paraspinal muscle layers lie superficial and lateral, framing the midline ligament. Clinically, the supraspinous ligament is a key posterior tension band that resists lumbar flexion, and it is often assessed in the setting of posterior ligamentous complex injury after flexion distraction trauma (Chance fracture). Palpable tenderness along the midline and widening of interspinous distance raise concern for associated disruption of the interspinous ligament and facet capsules, a pattern that changes stability and management. Its superior continuity with the nuchal ligament makes the cervical to thoracolumbar transition easy to teach as one functional sheet of midline connective tissue rather than isolated segments. A subtle but practical landmark.
