Description
Anterior palmar anatomy of the ulnar wrist is organized around the pisiform and hook of hamate, and the pisohamate ligament spans between them as a short, fibrous band on the volar-ulnar margin of the carpus. Proximal to the carpal row, the distal ulna and ulnar styloid sit medial to the distal radius, with the carpal bones arranged distal to the radiocarpal joint and the metacarpals projecting distally into the palm. Flexor tendons course longitudinally toward the digits, while the transverse and oblique palmar ligamentous fibers cluster around the carpometacarpal and metacarpophalangeal articulations. Small but unmistakable. Clinically, the pisohamate ligament forms part of the piso-hamate tunnel and blends with the tendon of flexor carpi ulnaris, a relationship that helps explain ulnar-sided wrist pain patterns and traction symptoms after a fall on the outstretched hand. The ulnar nerve and artery pass through Guyon canal just radial to the pisiform, so hypertrophy, ganglion formation, or post-traumatic changes around the pisohamate region can contribute to ulnar tunnel syndrome with sensory changes in the ulnar digits and intrinsic hand weakness. This anterior perspective clarifies why the hook of hamate and pisiform serve as palpable landmarks during surgical decompression or during ultrasound-guided evaluation of the ulnar neurovascular bundle.
