Description

Anterior palmar anatomy of the distal forearm and hand is presented with the palmar radioulnar ligament highlighted in blue at the ulnar aspect of the wrist. The ligament spans between the distal radius and distal ulna across the volar distal radioulnar joint, blending with the anterior joint capsule and the triangular fibrocartilage complex at the ulnar fovea and styloid region. Distal to the carpus, the metacarpals and phalanges align in the usual proximal to distal sequence, with the interphalangeal and metacarpophalangeal articulations shown in continuity. Flexor tendons course along the palmar surface toward the digits, lying superficial to the bony contours and reinforcing the sense of orientation. Palmar radioulnar ligament anatomy matters because it is a primary stabilizer against dorsal translation of the ulna and volar gapping at the distal radioulnar joint during forearm supination and pronation. Small tears at its TFCC attachment can produce ulnar-sided wrist pain, reduced grip strength, and a positive DRUJ ballottement test, even when plain radiographs look unremarkable. This anterior perspective helps when correlating MRI or wrist arthroscopy findings with the volar capsuloligamentous complex, and when planning TFCC repair or DRUJ stabilization where protecting nearby volar structures is non-negotiable.
