Description
Arising from the pisiform on the ulnar, palmar side of the wrist, the pisometacarpal ligament courses distally and laterally toward the base of the fifth metacarpal, bridging the transition from the proximal carpal row to the ulnar carpometacarpal region. From an anterior (palmar) perspective, the pisiform sits superficial and medial, while the hook of hamate lies more distal and radial as a neighboring landmark, and the ligament runs in the interval between them toward the ulnar metacarpus. The flexor carpi ulnaris tendon inserts onto the pisiform proximally, with the ligament continuing that line of force distally into the hand. Adjacent carpal and metacarpal cortices provide the bony context for this slender fibrous band. Functionally, this ligament represents the distal continuation of the pisiform complex and helps transmit flexor carpi ulnaris load to the ulnar border of the hand during grip and ulnar deviation. Pain and instability around the pisiform often get discussed as “pisotriquetral” problems, but the pisometacarpal ligament becomes relevant when you are teaching why symptoms can radiate into the hypothenar and ulnar carpometacarpal region and why surgical excision of the pisiform or injury to the flexor carpi ulnaris insertion can alter force transmission. In ulnar-sided wrist evaluations, it also frames the relationship between the pisiform and nearby structures such as the Guyon canal region, where the ulnar nerve and artery pass volar to the carpal bones. Small structure, real consequences.
