Description
Anteriorly, the palmar aspect of the male wrist and hand is opened to expose the proximal commissural ligament in relation to the flexor apparatus. Superficial and intermediate layers logically include the palmar aponeurosis with its longitudinal pretendinous bands, the flexor digitorum superficialis and profundus tendons running distally toward the digits, and fibrous transverse connections spanning between adjacent rays near the bases of the proximal phalanges and metacarpophalangeal joints. Digital neurovascular bundles course along the radial and ulnar sides of each finger, lying volar to the phalanges and deep to the skin but superficial to the flexor tendons within their fibrous sheaths. Layering matters. Understanding the proximal commissural ligament helps clarify how the palmar soft tissues maintain web space contour and resist splaying of the digits during power grip, while still permitting controlled abduction and adduction at the metacarpophalangeal joints. In burn reconstruction and post-traumatic scar management, contracture across the commissures can tether these fibrous bands and collapse the web spaces, creating functional loss that often requires Z-plasty or other commissuroplasty techniques that respect the underlying ligamentous and tendon relationships. For hand surgeons, the anterior view also frames safe dissection planes by keeping attention on the proximity of the digital nerves and arteries to the fibrous commissural tissue.
