Description
Palmar carpometacarpal ligaments span the volar aspect of the distal carpal row to the bases of the metacarpals, forming stout fibrous bands across the carpometacarpal (CMC) articulations. Proximally, the distal radius and ulna frame the carpus, with the scaphoid and lunate lying radial and central, then the trapezium and trapezoid laterally and the capitate and hamate more medially in the distal row. The highlighted bands typically course from the trapezium toward the first metacarpal base, from trapezoid and capitate toward the second and third metacarpal bases, and from hamate toward the fourth and fifth metacarpal bases, sitting anterior to the joint spaces and blending with the volar joint capsules. These are short, tough ligaments. A true anterior perspective on the palmar CMC ligaments matters because it clarifies why dorsal dislocation at the CMC joints is more common than volar dislocation: the volar ligament-capsular complex resists palmar translation while dorsal restraints may fail under axial load and hyperextension. The first CMC joint deserves special attention, where injury patterns and post-traumatic instability relate to the balance between the volar beak ligament complex and the dorsal ligament complex, a relationship that also frames surgical reconstruction decisions and arthroplasty planning in thumb CMC osteoarthritis.
