Description
Palmar carpal anatomy is presented from an anterior perspective, with the distal radius and ulna superior to the two carpal rows and the metacarpals extending distally into the digits. Across the volar surfaces of the scaphoid, lunate, triquetrum, pisiform, trapezium, trapezoid, capitate, and hamate, the palmar intercarpal ligaments form short transverse and oblique fiber bands that bridge adjacent bones and reinforce the intercarpal articulations. The thumb ray sits laterally with the trapezium and first metacarpal, while the hamate and fourth and fifth metacarpals lie medially, creating the ulnar border of the carpal arch. Subtle tendinous elements may be seen superficial to the carpus, but the emphasis remains on fibrous connective tissue spanning the carpal joints. An anterior rendering of these ligaments matters because volar ligamentous restraints are the primary checkrein to carpal translation and collapse when the wrist is loaded in extension, the position that commonly produces scapholunate injury and patterns of carpal instability. For hand surgeons and radiologists, this relationship helps explain why a tear of the scapholunate interosseous ligament often coexists with disruption of adjacent palmar capsuloligamentous fibers, contributing to dorsal intercalated segment instability and, over time, scapholunate advanced collapse. Small structures, big consequences. The palmar intercarpal ligaments also frame safe dissection planes during volar approaches to the wrist and guide placement of anchors or grafts in ligament reconstruction.
