Description
Centered within the male larynx, the paired vocal ligaments form the dense, elastic core of the true vocal folds, extending anteroposteriorly from the inner surface of the thyroid cartilage (near the anterior commissure) to the vocal processes of the arytenoid cartilages posteriorly. Inferior to each vocal ligament, fibers of the conus elasticus (cricovocal membrane) rise from the superior border of the cricoid cartilage, blending into the free margin that constitutes the vocal ligament. Lateral to the ligament sit the thyroarytenoid muscle (including the vocalis) and the paraglottic space, while the airway continues inferiorly into the tracheal lumen. Above the true folds lies the laryngeal vestibule and, depending on the model, the ventricular folds may appear superior and slightly lateral. Clinically, the vocal ligament is the layer most responsible for the sharp medial edge needed for efficient phonation, and its relationship to the conus elasticus explains why subglottic edema and scarring can stiffen the fold and alter the mucosal wave. Injury patterns also track the attachments: anterior commissure lesions behave differently from posterior glottic pathology near the arytenoids, and endotracheal intubation commonly produces ulceration or granulation at the vocal processes rather than the mid-membranous fold. For surgeons, the ligament marks the dissection plane in microlaryngoscopy and helps distinguish superficial lamina propria from deeper structural tissue during phonomicrosurgery.
