Description
Arising from the lateral surface of the zygomatic bone, the zygomaticus major sweeps inferomedially across the cheek toward the modiolus at the oral commissure, forming the dominant “smile line” as it blends with fibers of the orbicularis oris and neighboring perioral muscles. Lateral and slightly posterior to this straplike belly, the masseter occupies the ramus and angle of the mandible, while the zygomaticus minor sits superior and medial, running toward the upper lip. Superiorly, orbicularis oculi encircles the palpebral fissure, and inferiorly the facial musculature transitions toward the upper cervical region where the sternocleidomastoid becomes visible along the anterolateral neck. Fiber direction is explicit. A clean teaching view. Functionally, this arrangement clarifies how a single elevator of the mouth angle can dominate expression while remaining mechanically distinct from the muscles of mastication, a separation that matters when evaluating facial asymmetry after trauma or iatrogenic nerve injury. Because the zygomaticus major lies superficial to the buccinator and is traversed by distal branches of the facial nerve, it is a frequent reference point in parotid region and facelift dissections, and its relationship to the zygomatic arch helps orient the surgeon when protecting the zygomatic and buccal branches. Denervation produces a flat, lateral oral commissure and loss of smile excursion, a hallmark in Bell palsy grading and post-parotidectomy assessment.
