Zygomaticus Major Beneath The Skin

Running obliquely from the zygomatic bone to the modiolus at the corner of the mouth, the zygomaticus major is presented deep to the cheek’s subcutaneous tissue in a lateral view of an adult male head. Superior to it lie the orbicularis oculi fibers encircling the palpebral fissure, while inferiorly the orbicularis oris and adjacent perioral musculature frame the oral aperture. Posterior and inferior, the masseter overlays the mandibular ramus, and the temporalis occupies the temporal fossa above the zygomatic arch. A selected muscle is color-highlighted for teaching emphasis against the broader facial muscle field. Zygomaticus major is the primary elevator of the oral commissure, and this side-on orientation clarifies how its vector produces the characteristic superolateral pull of a smile while blending at the modiolus with risorius, levator anguli oris, and orbicularis oris. Surgeons care about that blending zone. It is the anatomic substrate for asymmetry in facial nerve (CN VII) palsy affecting the zygomatic and buccal branches, and it defines a target and a hazard region in midface rejuvenation, nasolabial fold procedures, and botulinum toxin placement that can flatten the smile if diffusion reaches the wrong fibers.
