Description
Positioned on the palmar aspect of the male hand, the annular (A) pulleys form stout transverse bands along the volar surfaces of the proximal and middle phalanges, where they encircle the flexor digitorum profundus and flexor digitorum superficialis tendons within the digital fibrous sheath. From this anterior, supinated perspective, the metacarpals, phalanges, and the carpal arch sit deep to the pulley system, while the flexor tendon pathways run longitudinally, centered over the phalangeal shafts. Proximal pulley structures blend with palmar fascia and neighboring retinacular fibers, creating a continuous restraint from the distal palm into each digit. Clear bony landmarks anchor the rings. Clinically, this view maps the mechanics that prevent tendon bowstringing during finger flexion, a failure most often discussed at A2 and A4 where pulley rupture follows forceful gripping or crimping and immediately changes tendon excursion and finger strength. Trigger finger (stenosing tenosynovitis) localizes to thickening at the A1 pulley near the metacarpophalangeal joint, and surgeons rely on the anterior approach to release A1 while protecting the digital neurovascular bundles that course along the radial and ulnar sides of each finger.
