Running in the anterior compartment of the leg, the paired venae comitantes of the anterior tibial artery ascend from the dorsum of the foot and ankle toward the popliteal fossa, positioned deep to tibialis anterior and the extensor hallucis longus and extensor digitorum longus tendons. From an anterior view, the vein’s course reads as a midline, deep channel along the shin, while the great saphenous vein tracks more superficially along the medial leg and crosses anterior to the medial malleolus. Distally, dorsal digital and metatarsal veins converge toward the dorsal venous arch and dorsal foot tributaries that feed the deep system through perforators. Venous valves appear as segmental interruptions along the column. An anterior perspective matters because it clarifies what is and is not superficial. The anterior tibial vein is not a cannulation target, but it is a frequent participant in isolated distal deep vein thrombosis, and its relationship to the interosseous membrane and anterior tibial neurovascular bundle (with the deep fibular nerve) frames the territory at risk in anterior compartment syndrome. For surgeons and interventionalists, this view also supports orientation during anterior approaches to the ankle and distal tibia, where retraction near the anterior tibial vessels can provoke bleeding or postoperative hematoma.
