Description
Running subcutaneously along the medial border of the foot and leg, the great saphenous vein is traced from the dorsal venous arch anterior to the medial malleolus, then ascending on the anteromedial tibia toward the knee and thigh. Proximally, its course continues in the medial thigh to the saphenofemoral junction, where it pierces the cribriform fascia at the saphenous opening to join the femoral vein. Major superficial tributaries are suggested along the calf and thigh, with deeper, more centrally placed channels indicating the paired tibial veins and popliteal-femoral pathway in the deep compartment. Valvular segments are implied along the long axis. Orientation is key. Clinically, this is the workhorse vessel for superficial venous disease: reflux at the saphenofemoral junction and along the trunk drives the common pattern of medial leg varicosities, edema, and venous stasis dermatitis. The anterior surface relationship to the medial malleolus also explains why the vein is frequently targeted for cannulation, cutdown, and ultrasound-guided access, and why it is vulnerable during medial ankle incisions. Mapping its superficial course against the tibia helps correlate physical exam findings with duplex ultrasonography, and it sets up the anatomic logic behind endovenous thermal ablation, cyanoacrylate closure, and surgical high ligation and stripping. Suitable for gross anatomy and vascular modules in medical curricula, this artwork also fits phlebology teaching decks, patient-facing procedural handouts, and textbooks covering chronic venous insufficiency and varicose vein interventions.
