Anatomical Structure and Location of the Great Saphenous Vein

Running superficially along the medial foot and leg, the great saphenous vein begins from the dorsal venous arch and medial marginal vein, courses anterior to the medial malleolus, then ascends along the medial border of the tibia toward the knee and thigh. Proximally, it tracks in the superficial fascia over the sartorius region and approaches the saphenofemoral junction at the groin, where it pierces the cribriform fascia to enter the femoral vein. A companion deep system is also represented, with paired venae comitantes of the anterior and posterior tibial veins converging to the popliteal vein and continuing as the femoral vein, clarifying superficial to deep relationships. Clinically, this long, continuous superficial conduit matters because it is the most common source of primary varicose veins, with valve incompetence producing reflux along the medial calf and thigh and visible tributary dilation. The depiction of valves and the superficial course relative to the deep veins supports teaching duplex ultrasound concepts, where perforator veins connect the great saphenous vein to the posterior tibial and other deep channels, and where mapping targets the saphenofemoral junction before endovenous thermal ablation, foam sclerotherapy, or surgical ligation and stripping.
