Description
Running superficially along the medial border of the male lower limb, the great saphenous vein begins from the medial end of the dorsal venous arch of the foot and ascends anterior to the medial malleolus, then continues up the medial leg and thigh toward the saphenous opening in the fascia lata. Short tributaries from the dorsum of the foot, the medial calf, and the anterior thigh typically join it, and venous valves are positioned to direct flow proximally against gravity. Deep to this superficial channel, the femoral artery and vein track in the femoral triangle and adductor canal, while the tibial arterial trunks and deep venous comitantes lie in the posterior and anterior compartments. Orientation against a semi-transparent skin and muscle context makes the superficial course easy to relate to palpable landmarks. Clinically, this vein matters because it is the principal conduit for superficial venous reflux and varicosities along the medial calf and thigh, often driven by incompetence at the saphenofemoral junction or by failure of valves in the truncal segment. Surgeons and interventionalists also rely on its predictable route when performing endovenous thermal ablation, ultrasound-guided foam sclerotherapy, or open high ligation and stripping, while staying alert to the nearby saphenous nerve along the medial leg.
