Description
Running along the medial side of the thigh and knee, the great saphenous vein tracks in the superficial fascia from the level of the groin toward the medial femoral condyle, accompanied by smaller venules and tributaries that converge as it ascends. Near the femoral triangle it angles anteriorly and superiorly to pierce the fascia lata at the saphenous opening, where it terminates at the femoral vein at the saphenofemoral junction. Superficial to the quadriceps femoris and adductor compartment contours, the vessel’s course contrasts with deeper venous channels that lie beneath the deep fascia and between muscle planes. A key landmark. Clinically, the segment where the great saphenous vein plunges through the fascia lata is central to both venous reflux assessment and operative planning, because valvular incompetence at the saphenofemoral junction is a common driver of primary varicose veins. This perspective also supports teaching of the perforator concept, since incompetent perforating veins around the medial knee and distal thigh can shunt high pressure from the deep system to the superficial system and worsen edema, skin change, and venous ulcer risk.
