Viewed from an inferior perspective, the axis (C2, epistropheus) presents a broad vertebral body with the dens (odontoid process) projecting superiorly from its anterior aspect, while the paired inferior articular processes sit posterolaterally, aligned to meet the superior articular facets of C3. Between the pedicles, the vertebral foramen opens centrally, bounded posteriorly by the laminae and bifid spinous process. Laterally, the transverse processes and transverse foramina mark the route for the vertebral vessels as they ascend through the cervical spine. Orientation from below is the clearest way to appreciate how the C2 to C3 zygapophyseal joints set cervical rotation and extension limits, and how facet alignment influences coupled motion in the subaxial neck. Clinically, this anatomy frames common trauma patterns, including C2 pars interarticularis fractures (hangman fracture) from hyperextension and distraction, and odontoid fractures at the base of the dens where vascular supply is tenuous and nonunion risk is higher. Small bony asymmetries matter here.
