Description

Seen from the plantar (inferior) aspect, the calcaneus sits posteriorly as the dominant heel structure, with the talus positioned superior to it at the subtalar region and the cuboid and navicular forming the midfoot bridge anterior to the hindfoot. Distal to the tarsus, the five metatarsals radiate toward the toes, their bases aligned proximally at the tarsometatarsal joints and their heads forming the transverse contour of the forefoot. The proximal, middle, and distal phalanges are arranged distally in each lesser toe, while the hallux shows only proximal and distal phalanges. Medially, the first ray and hallux define the medial longitudinal arch; laterally, the fifth metatarsal and cuboid outline the lateral column. A plantar bony view is the cleanest way to teach the architecture of the longitudinal and transverse arches because you can read the foot as two columns, talus to first metatarsal medially and calcaneus to fifth metatarsal laterally, tied together by the midfoot. That relationship matters in common problems such as pes planus and posterior tibial tendon dysfunction, where collapse of the medial column alters talonavicular alignment and forefoot abduction becomes apparent. It also frames clinical interpretation of midfoot injuries, including Lisfranc fracture dislocation at the tarsometatarsal complex and stress injury patterns across the metatarsal shafts.
