Description
Posterior thoracic anatomy is centered on the right and left main bronchi as they branch from the trachea and continue into lobar and segmental bronchi within each lung. Clustered at the hilar roots, the bronchopulmonary (hilar) lymph nodes lie adjacent to the bronchi and pulmonary vessels, receiving lymph from intrapulmonary lymphatic channels that track along the bronchovascular bundles. Green lymphatic vessels converge medially toward the root of each lung, positioned lateral to the mediastinum and posterior to the heart’s atria and great vessels that occupy the inferior midline of the field. Posterior orientation matters because it clarifies how lymph from the peripheral lung funnels centrally to hilar nodes before passing to tracheobronchial nodal stations and, ultimately, the bronchomediastinal trunks. That pathway underlies classic patterns of spread in bronchogenic carcinoma, where hilar and subcarinal involvement can precede supraclavicular metastasis, and it also explains why sarcoidosis and tuberculosis often present with prominent hilar lymphadenopathy. It is also the map bronchoscopists and thoracic surgeons carry when targeting nodal sampling, since the relationship of nodes to the mainstem and lobar bronchi determines the endobronchial ultrasound (EBUS) and mediastinoscopy corridors.
