Description
Posteriorly oriented, the distal large intestine is presented with emphasis on the pararectal lymphatics tracking along the rectal wall and mesorectal fat toward regional nodal stations. Green lymphatic vessels course superiorly from the anorectal region, with serial lymph nodes positioned along the pararectal chains and at the root of the mesentery, while adjacent blue vessels suggest accompanying venous channels within the same fascial planes. The appendix and cecum sit more laterally in the right lower quadrant, providing an anatomic contrast between midline rectal drainage and the more lateral ileocecal territory. Posterior mapping of these lymphatic pathways matters when you are staging rectal carcinoma, where patterns of nodal spread follow the mesorectal envelope and inform total mesorectal excision margins and radiotherapy fields.
