Posterior dissection of the male rectum highlights the lymphatic vessels within the rectal wall, with channels arising from the mucosa and muscularis propria and converging toward the mesorectum. Fine collectors course superiorly along the superior rectal vessels toward pararectal and inferior mesenteric lymph nodes, while additional pathways track laterally toward internal iliac nodal groups along the pelvic sidewall. Inferior drainage from the distal rectum trends toward the middle and inferior rectal region, with continuity toward internal iliac and, near the anorectal junction, potential communication with superficial inguinal nodes. Orientation is strict posterior, emphasizing relationships to the mesorectal fascia and presacral space. This view matters because rectal cancer staging and operative planning depend on predictable lymphatic routes and on the concept of the mesorectum as the primary nodal basin.
