Description
Posterior abdominal anatomy is presented with the large intestine draped around the retroperitoneum, and the colon lymphatics traced along the mesenteric border from cecum and ascending colon on the right, across the transverse colon, to the descending and sigmoid colon on the left. Collecting vessels converge on named nodal basins that follow the arterial supply, including epicolic and paracolic nodes along the colonic wall, intermediate nodes along the colic vessels, and principal nodes at the origins of the superior and inferior mesenteric arteries. Behind the bowel, the kidneys sit superior and lateral to the psoas major muscles, while the abdominal aorta lies slightly left of midline with the inferior vena cava to its right; lumbar plexus elements and prevertebral autonomic plexuses course in the same depth plane. That posterior perspective matters because it matches the operative field when the colon is mobilized off the posterior abdominal wall and mesocolon. Lymphatic drainage patterns explain the predictable spread of colon adenocarcinoma to pericolic and then central mesenteric nodes, and they also frame the extent of lymphadenectomy in right or left hemicolectomy, including the concept of high ligation at the vascular pedicle.
