Description

Cut away through the anterior skull, the paired orbits are opened to reveal the globes seated within the bony orbital cavities, with the optic nerves (CN II) coursing posteriorly toward the optic canals and the intracranial compartment. Within each orbit, the ophthalmic artery and its branches track alongside the nerve in the posterior orbit, while venous channels converge toward the superior and inferior ophthalmic veins that drain posteriorly toward the cavernous sinus. Extraocular muscles and their tendinous origins at the common tendinous ring sit posterior to the globe, framing the neurovascular bundle as it passes from the orbital apex into the cranial cavity. Spatially, the retina lies deep to the sclera and choroid within the globe, continuous posteriorly at the optic disc where the optic nerve exits. This cutaway orientation is the one you want when teaching orbital apex anatomy and the clinical consequences of confined space at the superior orbital fissure and optic canal. Optic neuropathy from orbital apex syndrome, compressive lesions, or traumatic retrobulbar hemorrhage makes more sense when the nerve, ophthalmic artery, and venous outflow are shown in close posterior relationship with little capacity to accommodate swelling. Venous drainage is not an academic footnote here, the ophthalmic veins provide a valveless route between the face and cavernous sinus, which underlies cavernous sinus thrombosis secondary to facial or paranasal infection. Use this artwork in gross anatomy and neuroanatomy teaching blocks, ophthalmology and ENT lectures on orbital cellulitis and cavernous sinus pathology, and surgical education on approaches to the orbital apex, optic nerve decompression, or management of retrobulbar hemorrhage.
