Description
Beneath the anterior nasal skin, the depressor septi nasi appears as a small, flat slip arising from the incisive fossa of the maxilla just superior to the central incisors and running superiorly to the mobile nasal septum and medial crura of the alar cartilages. In this anterior perspective, it sits on the midline, inferior to the cartilaginous septum and anterior to the maxillary alveolar process, with orbicularis oris immediately inferior and the paired levator labii superioris alaeque nasi positioned more laterally along the nasolabial region. Portions of the nasal bones and piriform aperture frame the superior extent, while adjacent facial musculature, including orbicularis oculi, zygomaticus major, buccinator, and masseter, provides orientation across the cheek and jaw. Functionally, depressor septi nasi couples upper lip movement to nasal tip position, depressing the nasal tip and narrowing the nares during smiling or active upper lip depression. Surgeons and injectors pay attention to this muscle during rhinoplasty and perinasal aesthetic procedures because an overactive depressor septi nasi can produce a dynamic drooping tip and shortened upper lip, and targeted myotomy or botulinum toxin can reduce that smile-related tip descent.
