Description
Lateral subcutaneous anatomy of the male midface is rendered with the depressor septi nasi emphasized beneath the nasal base, running superiorly from the maxillary incisive fossa and anterior nasal spine region toward the caudal nasal septum and the medial crus region of the alar cartilage. Superior to it sit the nasal bones and the bony piriform aperture, while the maxilla and dentition form the anterior facial buttress inferiorly and posteriorly. Nearby facial mimetic muscles are partially visible around the upper lip and ala, with the depressor septi nasi positioned deep to the skin and anterior to the nasal cavity and septal framework. Bony context extends into the upper neck, where the hyoid lies anterior to the cervical vertebrae. Surgeons and injectors pay attention to this muscle because it can pull the nasal tip inferiorly and narrow the nasolabial angle during smiling, a pattern often described as a dynamic drooping tip deformity. Small variations in its insertion onto the septum or alar margin help explain why some patients show marked tip depression with animation after rhinoplasty unless the muscle is released, reoriented, or selectively weakened.
