Description
Centered on the medial aspect of the distal thigh, the descending genicular artery arises from the femoral artery within the adductor canal and courses inferiorly toward the medial femoral epicondyle, where it divides into a saphenous branch and an articular branch to the periarticular genicular network. Anteriorly, it sits deep to sartorius and adjacent to the saphenous nerve, then sweeps toward the medial knee to contribute to anastomoses with the superior medial genicular and inferior medial genicular arteries. Larger companion channels in blue represent the deep venous return, tracking proximally toward the femoral vein while superficial pathways align more medially along the great saphenous route. Small arterioles branch distally to the proximal tibia and anterior knee capsule. Clear landmarks. This vessel matters because its periarticular supply helps maintain knee perfusion when the superficial femoral artery is occluded and collateral flow recruits the genicular anastomosis. Orthopedic and vascular surgeons also respect it during medial knee incisions and during harvest of a saphenous flap or saphenous vein, where the saphenous branch can be a source of bleeding or, conversely, a planned pedicle. In angiographic interpretation, recognizing the descending genicular contribution prevents mislabeling collateral channels as pathologic neovascularity. Use this illustration in lower limb anatomy teaching to connect the adductor canal contents with the medial knee vascular arcade, and in clinical education materials explaining collateral circulation in femoropopliteal disease or planning medial parapatellar and pes anserinus approaches.
