Description
Seen from a posterior (dorsal) perspective, the male wrist and hand are opened to the level of the metacarpals and phalanges, with the dorsal carpal arch spanning transversely across the dorsum of the carpus. Radial and ulnar contributions approach the arch from the lateral (radial) and medial (ulnar) sides respectively, then give rise to dorsal metacarpal arteries that course distally in the intermetacarpal spaces toward the dorsal digital arteries of the fingers. Superficial dorsal veins run in blue over the extensor surface, while the carpal bones, metacarpals, and phalanges provide the bony framework deep to the vascular network. Dorsal carpal arterial anatomy matters when planning or interpreting procedures that work across the dorsum of the wrist, where small-caliber vessels can be injured or mistaken for tendinous structures. A common teaching point is the close relationship between dorsal wrist incisions and branches of the radial artery and dorsal carpal arch, relevant in open reduction and internal fixation of distal radius fractures and in dorsal approaches to the radiocarpal and midcarpal joints. Bleeding here is often brisk.
