Seen from the dorsal (posterior) aspect, the hand is presented with the phalanges and metacarpals as a bony scaffold for the superficial vascular network. Dorsal metacarpal arteries course distally along the intermetacarpal spaces and divide into dorsal digital arteries that run on the dorsolateral margins of the proximal phalanges, paired on adjacent sides of neighboring fingers. Proximally, the vessels relate to the distal carpal row and dorsal carpal arch, with contributions consistent with radial and ulnar arterial systems at the wrist. Color separation distinguishes arteries from accompanying superficial dorsal venous channels. This posterior perspective matters because dorsal digital arteries behave differently from the palmar digital arteries you protect in volar approaches, yet they are frequently encountered in dorsal incisions, flap design, and trauma exposure. Dorsal lacerations over the proximal phalanx can compromise these longitudinal vessels and their perforating connections to the palmar system, influencing fingertip perfusion when combined with edema or crush injury. A clean map of their course also supports teaching the dorsal venous network and the relative paucity of protective soft tissue over extensor tendons and dorsal capsules.
