Seen from the dorsal (posterior) aspect, the hand and wrist are rendered with the metacarpals and phalanges as bony landmarks and a red arterial tree coursing superficially across the dorsum. Dorsal carpal branches from the radial and ulnar arteries contribute to the dorsal carpal arch over the proximal carpus, giving rise to dorsal metacarpal arteries that run distally in the intermetacarpal spaces and divide into dorsal digital arteries along the sides of the fingers. Blue dorsal venous channels parallel the arterial paths more loosely, converging proximally toward the dorsal venous network over the metacarpals. A posterior perspective is where the balance between superficial soft tissue and exposed vascular landmarks becomes clinically legible, since the dorsal hand has relatively thin subcutaneous coverage and the dorsal veins and accompanying arteries sit close to extensor tendons and metacarpal shafts. This is the view you want when teaching arterial anastomoses that maintain perfusion across the wrist, and when discussing why lacerations or crush injuries over the dorsum can compromise dorsal metacarpal arteries and produce distal digital ischemia despite an intact palmar arch. Small vessel caliber matters here. Even minor edema or hematoma can distort these dorsal channels.
