Description
Oriented to an inferior aspect of the brainstem, the basilar artery is centered along the ventral surface of the pons, giving rise to multiple small pontine arteries that pierce the pontine parenchyma in a roughly transverse, segmental pattern. Medially, paramedian perforators course a short distance before entering near the basilar sulcus, while more laterally directed circumferential branches sweep toward the anterolateral pons where they anastomose with territories supplied by the anterior inferior cerebellar artery. A sharp midline reference. The ventral pons lies superior to the clivus in situ, with the cerebellar peduncles and adjacent pontomedullary junction forming key inferior landmarks. Understanding the pontine perforator anatomy matters because these vessels behave like end arteries, and small changes in patency translate into disproportionate neurologic deficits. Basilar artery thrombosis, perforator occlusion, and hypertensive lipohyalinosis can produce classic medial pontine syndromes and lacunar infarcts, while endovascular stenting or thrombectomy across the basilar trunk risks “snowplowing” plaque into the perforator origins. This inferior viewpoint mirrors the operative and angiographic mental model used when localizing lesions to the ventral pons and anticipating which long tracts and cranial nerve nuclei lie at risk. Neuroanatomy and neuroradiology teaching sets can pair this artwork with axial MRI or CTA to map perforator territories onto clinical signs such as contralateral hemiparesis, dysarthria, and gaze disturbances.
