Description
Running along the anterolateral tibia, the extensor hallucis longus (EHL) forms a narrow muscle belly that originates from the middle third of the fibular shaft and interosseous membrane, then tapers into a tendon that crosses anterior to the ankle toward the dorsal foot. Medial to it lies tibialis anterior, while extensor digitorum longus sits more laterally, with the fibula marking the lateral boundary of the anterior compartment. Distally, the EHL tendon passes deep to the superior and inferior extensor retinacula and continues to the dorsal base of the distal phalanx of the hallux, with the anterior tibial vessels transitioning into the dorsalis pedis artery in close company on the dorsum of the foot. Bony context includes the tibia, fibula, ankle mortise, metatarsals, and phalanges. An anterior presentation of EHL matters because it is the cleanest way to teach the relationship of dorsiflexor tendons to the extensor retinacula and to the neurovascular bundle of the anterior compartment, especially the deep fibular (peroneal) nerve traveling with the anterior tibial artery between tibialis anterior and EHL. Weak great toe extension is a useful bedside clue. It can indicate deep fibular neuropathy, L5 radiculopathy, or tendon disruption after an anterior ankle laceration, and the tendon’s course across the tibiotalar joint helps explain pain and snapping following retinacular injury or postoperative scarring. The same anatomy frames anterior compartment syndrome, where rising pressure threatens the deep fibular nerve and dorsalis pedis perfusion. Orthopedic and sports medicine texts use this view to illustrate EHL tendon repair, ankle arthroscopy portals, and safe planes for dorsal foot incisions. It also fits lower limb anatomy labs and radiology teaching files when correlating surface anatomy with ultrasound or MRI of anterior ankle tendons.
