Description
Arising from the anterior surface of the fibula and adjacent interosseous membrane, the extensor hallucis longus (EHL) runs distally in the anterior compartment, positioned deep to tibialis anterior and medial to extensor digitorum longus before its tendon becomes prominent as it approaches the ankle. From this lateral perspective, the fibula lies lateral to the tibia, while the EHL tendon passes anterior to the tibiotalar joint beneath the superior and inferior extensor retinacula on its way toward the dorsum of the hallux. Peroneus (fibularis) longus and brevis occupy the lateral compartment posterior to the fibula, contrasting with the anterior extensor tendons that course toward the dorsum of the foot. The Achilles tendon anchors posteriorly at the calcaneus. Clean landmarks. Functionally, EHL is the prime mover for interphalangeal extension of the great toe and a contributor to ankle dorsiflexion, so its tendon line of pull becomes a useful teaching point when comparing toe extension with tibialis anterior driven dorsiflexion. Clinicians often assess hallux extension strength when screening for deep fibular (peroneal) nerve compromise or L5 radiculopathy, and this side view clarifies why anterior compartment swelling can preferentially affect EHL and its neurovascular neighbors.
