Description
Running along the posterior compartment of the leg, the flexor hallucis longus arises from the inferior two thirds of the posterior fibular shaft and adjacent interosseous membrane, then descends deep to the soleus toward the ankle. From a lateral perspective, the fibula lies lateral to the tibia, while the muscle belly sits posterior to the fibula and narrows into a tendon that tracks inferiorly toward the hindfoot. At the ankle, the tendon courses posterior to the talus and calcaneus, then continues into the foot to reach the hallux, inserting on the base of the distal phalanx. Bone alignment is clear. This lateral orientation matters because the flexor hallucis longus tendon is a frequent pain generator in posterior ankle impingement and stenosing tenosynovitis, where it can be irritated as it glides through the fibro-osseous tunnel posterior to the talus and under the sustentaculum tali. The illustration also supports teaching of the tendon’s role in push-off and hallux interphalangeal flexion, and why an os trigonum or enlarged posterior talar process can provoke symptoms in dancers and soccer players during forced plantarflexion.
