Description

Running superficially across the lateral neck, the external jugular vein (EJV) descends obliquely over the sternocleidomastoid from the region just inferior to the angle of the mandible toward the clavicle. Superiorly, tributaries consistent with the posterior auricular vein and posterior division of the retromandibular vein converge near the parotid region, while smaller superficial cervical veins form a visible network along the posterolateral neck. At its inferior end, the EJV approaches the posterior triangle and courses toward the root of the neck to terminate in the subclavian vein deep to the platysma and investing layer of deep cervical fascia. Half-surface, half-transparent. That lateral relationship to the sternocleidomastoid is why clinicians care about the EJV, it is the most accessible superficial venous channel for estimating jugular venous pressure when internal jugular assessment is limited, and it can become strikingly distended in right-sided heart failure, pericardial tamponade, or superior vena cava obstruction. The course over the muscle also explains vulnerability in penetrating trauma and why air embolism remains a concern when the vein is lacerated above the clavicle, where negative intrathoracic pressure can entrain air.
