Running up the anteromedial thigh, the femoral vein is shown in longitudinal course deep to the fascia lata, continuing proximally toward the femoral canal and distally into the adductor canal. At the level of the femoral triangle it lies medial to the femoral artery, with the femoral nerve positioned lateral to the artery, a relationship that stays consistent until the vessels pass beneath the sartorius. Bony landmarks such as the femur, patella, tibia, and fibula anchor the vascular anatomy, and the knee joint region is included to orient the transition into the popliteal segment posterior to the joint line. For clinicians, this anterior arrangement matters because it governs access and risk. Common femoral venous catheterization relies on palpating the femoral artery and cannulating just medial to it, while avoiding high puncture near the inguinal ligament that increases retroperitoneal bleeding risk and low puncture that increases arteriovenous fistula and pseudoaneurysm rates. Deep vein thrombosis frequently involves the femoral vein, and appreciating its caliber changes and its confinement within the femoral sheath helps explain why thrombus can propagate proximally with embolic potential.
