Description
Anteriorly, the first thoracic vertebra (T1) is presented with the vertebral body centered and the superior and inferior endplates framing the intervertebral disc spaces. A single, full costal facet lies on each superolateral margin of the body for articulation with the head of rib 1, while the inferior demifacet for rib 2 sits slightly lower and lateral at the body margin. Pedicles project posteriorly from the body to form the vertebral arch, and the transverse processes extend laterally toward their transverse costal facets (for the tubercle of rib 1), though these are only partially appreciated from a strict anterior perspective. Subtle superior lipping along the vertebral body rim is visible. A boundary vertebra. T1 matters because it marks the cervicothoracic junction, where cervical lordosis transitions to thoracic kyphosis and where anatomy shifts from transverse foramina and bifid spinous processes to costal facets for rib articulation. Trauma at this level often accompanies first-rib fractures, and the nearby thoracic inlet makes fractures and dislocations clinically charged, with potential involvement of the sympathetic chain (Horner syndrome) or adjacent neurovascular structures. From an anterior view, the costal facet arrangement and body morphology help distinguish T1 from C7, a frequent point of confusion on plain radiographs and CT. Use this artwork in spine anatomy teaching to anchor thoracic vertebral identification by rib facet patterns, in radiology teaching files to support cervicothoracic level localization, and in surgical education when orienting approaches near the thoracic inlet and upper thoracic instrumentation planning.
