Description
Seen from an inferior aspect, the occipital bone forms the posterior skull base around the foramen magnum, with a fracture line tracking along the right posterolateral rim and extending into the adjacent squamous and condylar regions. The occipital condyles sit anterolateral to the foramen magnum, flanked by the hypoglossal canal superior to each condyle and the condylar canal posteriorly. Laterally, the occipital bone meets the petrous temporal bone at the petro-occipital region near the jugular foramen, while the basilar part of the occiput continues anteriorly toward the sphenoid. Sharp margins. Clear disruption. Fractures at the margin of the foramen magnum raise immediate concern for injury at the craniovertebral junction, where the occipital condyles articulate with the superior articular facets of the atlas and transmit forces to the upper cervical spine. A rim-spanning occipital fracture can coexist with occipital condyle fracture patterns (often best characterized on CT with sagittal and coronal reformats) and may threaten nearby neurovascular structures, including the lower cranial nerves traversing the jugular foramen and the vertebral artery as it courses toward the dura. Clinically, this anatomic neighborhood explains symptoms ranging from occipital pain and restricted rotation to dysphagia, hoarseness, or hypoglossal palsy when fracture propagation involves the hypoglossal canal.
