Description
Running in the superficial fascia from the medial dorsum of the foot to the groin, the great saphenous vein ascends anterior to the medial malleolus, then courses proximally along the medial border of the tibia and across the medial knee toward the anteromedial thigh. Its terminal segment tracks toward the saphenofemoral junction in the femoral triangle, where it pierces the fascia lata at the saphenous opening to join the femoral vein. Surrounding landmarks include the tibia and fibula in the leg, the patella at the knee, and the femur in the thigh, with anterior compartment musculature such as tibialis anterior shown deep to the vessel. Long and superficial. Anterior views like this are the clearest way to teach surface anatomy and the clinical map used for venous access and varicose vein assessment, because the great saphenous vein stays reliably medial while deeper neurovascular bundles remain compartmentalized beneath the deep fascia. Reflux at the saphenofemoral junction and incompetent perforators along the medial calf and thigh commonly produce tortuous varicosities and venous stasis changes, so appreciating this proximal termination and its relationship to the fascia lata matters when planning endovenous ablation, high ligation and stripping, or phlebectomy. The medial malleolus segment is also a familiar landmark for ultrasound-guided cannulation or for tracing the vein’s course in preoperative vein mapping for coronary artery bypass grafting. Use this plate in lower-limb anatomy teaching sessions, vascular surgery and phlebology lecture slides, and clinical skills materials covering venepuncture and duplex ultrasound orientation of superficial venous trunks. It also fits well in patient-facing brochures explaining varicose vein procedures, where a straightforward anterior course supports informed consent discussions.
