Description
Over the lateral shoulder, the deltoid fascia forms a fibrous sheath investing the deltoid muscle and contributing to the rounded contour of the male shoulder. The deltoid is organized into anterior (clavicular), middle (acromial), and posterior (spinal) parts, with fibers converging distally toward the deltoid tuberosity on the lateral humeral shaft. Superiorly, the fascia blends with attachments along the clavicle, acromion, and spine of the scapula, sitting superficial to the subacromial region and the rotator cuff tendons. Posteriorly and medially, neighboring planes include trapezius over the scapular spine and the deeper supraspinatus and infraspinatus muscles approaching the greater tubercle. Fascial anatomy at the shoulder matters when you are planning surgical approaches and injections because tissue planes, not just muscles, determine dissection resistance and the way fluid spreads. Deltoid-splitting and deltopectoral approaches both intersect regional fascia, and understanding its continuity with periosteal attachments helps explain why deltoid detachment or aggressive retraction can compromise abduction strength or contribute to postoperative pain. A clear fascial envelope also frames common clinical teaching points such as safe intramuscular deltoid injection placement, staying lateral and distal enough to avoid the axillary nerve as it courses deep to the deltoid around the surgical neck of the humerus. Good landmarks. Few are more practical.
